About the Role
- You will be responsible for assessing the patients' blood work, requisitioning appropriate blood work from assessment, analyzing results, and collaborating with a team of health and wellness professionals to design personalized healthcare plans that promote long-term health and wellness.
- All patients who request this care will have a primary care provider in place for the NP to refer patients back to regarding their diagnostic results.
- Virtual care, diagnostic testing, and telehealth experience is a must..
What you’ll do:
Longevity Healthcare Planning:
- Develop and implement individualized care plans based on the patients' bloodwork and Longevity score, while focusing on longevity and preventive care.
- Educate and counsel patients on risk factors as well as nutrition, exercise, sleep best practices to improve longevity
- Requisition blood work to Dynacare
- Interpret results and follow up with patients
- Refer back to their primary care provider
Virtual Healthcare Delivery:
- Utilize PHIPPA approved telehealth platforms to consult with patients, providing accessible and efficient care.
Collaboration & Teamwork:
- Work closely with physicians, nutritionists, and other health care professionals to provide a multidisciplinary approach to patient care.
What you’ll need:
Background
- Masters degree in Nursing.
- Current registration as a Primary Care Nurse Practitioner in Ontario.
- Certification in a relevant specialty area is an asset.
Skills:
- Strong clinical skills with a focus on longevity and preventive medicine.
- Proficiency in using telehealth technologies and electronic health records.
- Excellent communication and client-facing interpersonal skills.
- Compassionate and patient-centered approach.
- Ability to work independently and as part of a team.
Experience
- Minimum of 2 years’ experience as a Nurse Practitioner, preferably in a primary care or private health setting.
- Experience working in multidisciplinary teams and with virtual healthcare delivery.
Compensation
$75.00 - $85.00 per hour
